Shoulder pain can be life-altering, making everyday activities challenging and impacting your overall well-being. If you’re considering shoulder replacement, it’s essential to understand the process from start to finish. Dr. Steven S. Goldberg, a renowned orthopedic surgeon and shoulder specialist in Naples, Florida, has helped thousands of patients regain mobility and live pain-free. In this blog, we’ll walk you through the comprehensive shoulder replacement journey, from pre-surgery preparation to post-surgery recovery, and answer key questions for prospective patients.

Stages of the Shoulder Replacement Process

1. Initial Considerations

Many patients consider a visit to the orthopedic surgeon for chronic pain in their shoulder that will not go away. Other patients have been referred by another doctor who has already diagnosed them with advanced shoulder arthritis. Patients also may have been told they need surgery and would like a second opinion. We see all of these types of patients on a regular basis and are happy to evaluate and discuss. Sometimes, patients just find reassurance in speaking to providers experienced in this field. Our office performs the highest volume of shoulder replacement surgery in Lee and Collier Counties in Southwestern Florida.

2. Pre-Surgery Preparation

The journey begins with a thorough evaluation, including medical history, physical examination, and imaging studies, which usually consist of an X-ray and often a CT scan or MRI. Dr. Goldberg and his team will discuss your treatment options, whether you’re a candidate for partial shoulder replacement, total shoulder replacement, or reverse shoulder replacement. If we decide that surgery is the most appropriate treatment, you’ll receive guidance on pre-surgery exercises, medication management, and what to expect on the day of surgery.

3. Pre-Operative Medical Evaluation

All patients undergoing shoulder surgery will have a basic medical workup and a visit with their primary doctor and routine bloodwork will be performed. This is to screen patients and ensure your health is optimal before undergoing surgery and anesthesia. In some cases, additional specialists, such as a cardiologist or a pulmonary doctor, will also evaluate patients with specific conditions. While this seems cumbersome, we do this to ensure the highest level of safety for patients undergoing surgery.

4. Right Before Surgery

In the few days before surgery, your doctor will explain which medications to stop and how long before. For instance, some blood thinners need to be stopped three days prior to surgery. Aspirin should normally be stopped one week before surgery. Most other medications can be taken on your normal schedule, and we will discuss this with you during your pre-surgery visit.

5. The Day of Shoulder Surgery Procedure

You will not eat breakfast or any food or coffee on the morning of your surgery. You can take your allowed medications with a few sips of water. You will be told when to arrive at your hospital or surgery center, and this may be early. Once you are brought into the pre-operative area, you will be asked a bunch of questions by the nurses, and soon after, will be given medicine to relax you before anesthesia is administered. Once you are in the operating room and the procedure begins, Dr. Goldberg performs your procedure in a less-invasive manner than traditional techniques, often using the Catalyst shoulder devices that he developed and has utilized for nearly a decade. The procedure involves replacing the damaged parts of your shoulder with artificial components designed to restore function and relieve pain. Dr. Goldberg utilizes minimally invasive techniques whenever possible to reduce pain and speed up recovery, ensuring a precise and effective operation tailored to your unique needs.

6. Right After Surgery

Many patients who have shoulder replacement surgery go home the same day. Others will stay overnight in the hospital, usually one night. This depends on your specific condition and medical history. In either case, you will be awake and comfortable within a few hours. Most people can eat and drink that day. We encourage you to walk around and you are able to shower because of the waterproof bandage placed over your incision. If you are at home, most patients feel more comfortable sleeping somewhat upright in a lounge chair, on the couch or if they can elevate the head of their bed. We often arrange a nurse to visit you at home the day after for a quick check-in. Most patients can use their operative arm the next day for simple activities, such as texting, typing, and eating.

7. Post-Surgery Recovery

One week after surgery, most patients come back in for a visit where the bandage is taken off and sutures are removed. Many of our patients already report feeling better than before surgery, this early. Patients typically begin gentle physical therapy soon after surgery to regain strength and mobility. In certain circumstances, some patients are encouraged to just do this at home themselves initially. Dr. Goldberg’s team provides a personalized rehabilitation plan and ongoing support to help you achieve the best possible outcome. We offer physical therapy services within our office, as well as working wth many of the local therapy providers. Therapy typically is twice a week and can last until about three months from surgery. Most patients can expect significant pain relief in the first several weeks, and improved function within two to three months. By three months, many patients are able to resume all of their normal activities, including sports.
